- Aadhaar number or Aadhaar enrolment number. If Aadhaar has the same address, no extra address proof needed.
- Apply in Form IV through the UDID portal.
- Certificate issued in Form V or VI with UDID in Form VII.
- If rejected, reasons in Form VIII with right of appeal.
- Issue within 3 months where disability is diagnosed. Set permanent or time-bound validity as appropriate.

UDID number is mandatory for several benefits like repeat ADIP, scholarships, and fee concessions. For some distributions, UDID enrolment is acceptable as specified in the latest O.M.
Where do applicants apply and what forms are used

What is the timeline to issue certificates

How do we compute multiple disability percentages
Assess each disability with the 2024 notified guidelines. Include only those that are 25 percent or more. Combine using the notified formula until a final composite percentage is obtained, capped at 100 percent.
