About this screen
- PHQ-4 combines GAD-2 and PHQ-2. It checks for anxiety and depressed mood over the last 2 weeks.
- Each item is scored 0 to 3. Subscales: GAD-2 = items 1–2, PHQ-2 = items 3–4.
- Total score 0–12: 0–2 normal, 3–5 mild, 6–8 moderate, 9–12 severe. Positive subscale screens if GAD-2 or PHQ-2 is 3 or more.
- Positive screens should be followed by longer tools such as GAD-7 and PHQ-8 or PHQ-9, and a clinical evaluation.

What is the PHQ-4?
PHQ-4 is a brief screen made of two anxiety questions and two depression questions. It offers a fast view and helps decide if longer checks are needed.
How are the scores used?
You get a total score from 0 to 12 and two subscale scores. GAD-2 or PHQ-2 scores of 3 or more are positive screens. Total bands are normal, mild, moderate, and severe.
Is this a diagnosis?
No. PHQ-4 is a screening tool. Only a licensed clinician can diagnose and suggest treatment.
